ETIAS is required for short stays in 30 European countries: the 29 Schengen members plus Cyprus. That includes France, Germany, Spain, Italy, the Netherlands, Portugal, Greece, Austria, Belgium, Switzerland, Norway, Sweden, Denmark, Finland, Poland, Czechia, Hungary, Croatia, and the micro-states accessed through them.
No. Ireland is in the EU but outside Schengen and ETIAS, with its own entry rules. United Kingdom is outside ETIAS and running its own ETA scheme. You do not need ETIAS for either.
No. One approved ETIAS covers all 30 participating countries for the 3 years it is valid. You apply once and can move between them, subject to the 90-days-in-180 limit across the whole area combined.
In practice yes. Monaco, San Marino, Vatican City, and Andorra have open borders with surrounding Schengen countries, so you enter and leave them through a participating country where ETIAS and EES apply.
